When my husband has an orgasm, the skin directly over his heart gets blotchy
red and has goose bumps. He is 40 in good health. I have not mentioned this
to him because I didn't want to cause him undue alarm if this is perfectly
normal. Do all men do this during orgasm, or should we be concerned?

What you are referring to is almost certainly a phenomenon known as "sex
flush." Although more common in women, many individuals develop redness or
even small bumps on their chest during heightened sexual arousal and/or orgasm.
This is nothing more than a manifestation of vasocongestion -- the rush of
blood to the genitals and other extremities during sexual arousal. There is
no reason to be alarmed. In fact, you may well have a similar reaction and
just never noticed it. Regardless, the flush should pass within a few hours
following sex if not sooner.
