The ball ring is too small but with a little lube the suction is plenty enough to hold it on. It doesn't sag at all so you're in good control of it. I gave it 4 stars because it creates so much suction in certain positions that it caused pain and gave me these little "hickeys" on the head of my penis. So now, if I feel any pain I stop and re-attach the extension so there is an air barrier between the end and my head. Overall a great product with room for development.
